Learning objectives:

1. Provide three reasons for the growing trend toward home and community-based care.

2. Discuss 2017 updates in services coordinated and monitored by CICOA Aging and In-Home Solutions as well as the updates in eligibility criteria for State funding.

3. Articulate the entire process and timeframe for establishing HCBC for a client - from identifying an appropriate consumer and making a professional referral to the local Area Agency on Aging, to having client services in place.

About the Presenter

Kate Kunk, RN, CIRS-A/D, holds degrees in nursing and sociology, as well as a certificate in gerontology.  She is currently working toward a certificate in geropsychology and is pursuing specialization in the area of Faith Community Nursing.  Kate coaches family caregivers on behalf of Indiana’s Area 8 Agency on Aging, (CICOA Aging & In-Home Solutions).  Before joining CICOA, her skills in advocacy have taken her from homeless shelters in New York City and Virginia’s Roanoke Valley—to a psychiatric clinic in Tennessee’s Great Smoky Mountains. She also worked in the publishing industry for more than three decades, developing college textbooks and other educational materials for McGraw-Hill and Pearson in the New York metropolitan area. Reducing the incidence of serious preventable illness and injury, enabling all of us to achieve and maintain optimal quality of life, are among Kate’s lifelong passions.
