Learning objectives:

1. To have a better understanding of managing chronic diseases in the home environment and efficiently refer patient to appropriate agencies.

2. Assist in educating patient and caregivers to have more realistic expectations in a home setting.

3. To be able to manage and utilize community resources efficiently and maximize the benefit the patient can gain from it.


 

About the Presenter

Alvin Reyes is a graduate of BS Physical Therapy from the Lyceum Northwestern University in 1993; a licensed physical therapist in the state of Indiana, New York, and Illinois; and has a certification from the American Institute of Balance-Vestibular Assessment and Management. On top of being a clinician, he held various managerial positions such as a clinical supervisor in the national kidney institute of the Philippines (now National Kidney Transplant Institute), director of rehab in Haborside Healthcare and Mulberry Health and Retirement Community, and administrator in Mulberry Home Health Services and a PT supplemental at IU North Hospital. As of today, he is the therapy director in 1st Care Home Health Services, having accumulated more than 25 yrs in different healthcare setting. His extensive experience in clinical practice and proven managerial leadership, won him the Harborside Healthcare-Midwest Region's Prestigious Award Winner: Director of Rehab of the Year 2002. 

This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers.  The course is approved for two CE contact hours.
